[CCHS-2009] CLARIFICATION - PROM GUEST FORM

As noted in an an earlier email, if your son or daughter is planning 
to bring a prom guest from another school, a permission form must be 
completed and signed by an authorized administrator of the guest's school.

Just to clarify - this type of permission form is typically signed by 
a principal, assistant principal or in private schools, a headmaster, 
dean of students or academic dean. Most schools in our area are very 
familiar with this type of request and the main office at the guest's 
school can assist in getting the proper signature.

These forms are available in the main office or by emailing Cathy 
Galligan (catgalligan at comcast.net) and must be returned to Mr. Miller 
prior to buying prom tickets. This is a new procedure this year and a 
ticket may not be purchased for someone who does not attend CCHS 
without the completed form.

Prom tickets will be on sale next week (5/5-5/9) during all lunch 
blocks in the cafe. The price is $60/ticket and checks may be made 
payable to "CCHS Class of 2009". Dates are certainly NOT required to 
attend the prom.
